Usually, only the inferior turbinates need correction.rnrnLarge turbinates can cause nasal obstruction. In some cases where a deviated septum is present, compensatory hypertrophy of the turbinate occurs on the side opposite the deviation. The nose does its job of filtering, warming and humidifying when the oncoming airstream is about 2-3 millimeters wide. If the airway is too large on one side, the turbinates compensate by getting bigger. That is why turbinate surgery is sometimes necessary when the septum is straightened. Sometimes your doctor will recommend turbinate surgery if medication is not effective in opening your air passages.rnrnMost turbinate surgery is performed under general anesthesia in the operating room. Special instruments are used to remove bone inside the turbinate or the fleshy outer portion is removed. Since the surgical area is not closed with sutures, temporary packing mad of absorbable collagen is placed. This packing will either disintegrate or fall out after a few days. Most patients go home the same day. However, accurate and secure flap approximation is also fundamental to a satisfactory outcome. When vertical wound tension prevents gentle flap approximation, a midline subcutaneous suture of 6-0 Monocryl is often necessary to approximate the flap edges. This deep-layer closure shifts tension to the subcutaneous tissues and prevents gradual widening and inversion of the final scar. Surface alignment is then accomplished at each flap corner with a simple interrupted suture. Surface sutures are placed close to the wound edge at superficial depth to ensure precise edge alignment. rnrnAttention to detail and meticulous technique are critical to satisfactory scar camouflage. The shield graft was fixated to the intermediate crura and dome and a bruised onlay graft and columellar onlay graft were placed caudal to the shield graft with 5-0 PDS horizontal mattress sutures. Combined lateral crural strut grafts/rim grafts were placed on the posterior caudal surface of the lower lateral cartilage and fixated with 5-0 PDS. Bruised cartilage was placed around the superior edges of the shield graft.
